Request a Practice Review

A practice review involves an on-site evaluation by a TMLT risk management professional to help determine your risk exposures. Specifically, the review will: examine the office for physical safety concerns
review practice policies and procedures
evaluate medical record documentation
provide follow up through a confidential, written summary

Practice reviews are free to all TMLT policyholders and can be completed without disrupting patient appointments. Policyholders who complete the process may be eligible for a 5% premium discount, after review recommendations are met.

By completing this registration form, your information will be submitted to the risk management department and you will be contacted to schedule a practice review.

Practice review preparation

We prefer to conduct practice reviews during a normal working day with regular patient scheduling. Please keep this in mind when scheduling your practice review.

Review your own risk management techniques in advance by completing a pre-practice review questionnaire. The questionnaire covers:
staffing
appointments
telephone protocol
termination of physician/patient relationship
patient complaints
release of medical records/information
procedure for follow up
medication administration
patient visits
policy and procedures for exams and treatments
diagnostic procedures
medical records
informed consent

If you are requesting CME credit, you will be sent a separate CME questionnaire in advance. During the practice review, you must meet with the representative for a practice review summation interview to be eligible to receive CME credit.
