How does your practice measure up?

Policies and procedures

We have written guidelines for staff, including topics such as telephone triage, management of emergencies in the office, and medication refills.
We have written protocols regarding the scope of care given by mid-level practitioners (PA-Cs, APNs) who are a part of our practice.
Our staff have job descriptions to identify duties and avoid exceeding them.

Documentation

Medications and allergies are updated in patient medical records at each visit.
All telephone calls, including prescription refills and after-hours calls to the physician(s), are documented accurately in the medical record.
Informed consent for both in-office and surgical procedures is appropriately documented.
Patients' informed refusal of tests or treatment is documented in the medical record.

Follow up

We have a tracking system that insures all results are received, including lab reports, diagnostic testing, and consultant notes. If results are not received, we have a back-up system to alert us and to prompt follow up.
If a patient cancels an appointment without rescheduling or misses a scheduled appointment, we have a system to document and follow up with the patient.

Termination of the physician/patient relationship

We feel confident that our protocol to terminate a physician-patient relationship is adequate.

Medical records maintenance

Medical records are maintained for the legally required length of time, and are safely destroyed when appropriate.
