Trust Rewards Program — Enrollment form
You have two options for submitting your Trust Rewards Enrollment form to TMLT.
1. Online enrollment with electronic signature
This enrollment form requires an electronic signature. To complete the form, you must create an account with TMLT. Once completed, the form will be automatically submitted to TMLT.

2. Enrollment with printable PDF
If you do not want to create an account and submit the form with an electronic signature, please use the fillable PDF form. Once the form is complete, you will need to print it, sign it, and return it to TMLT.
