Application Kit

1. Complete and sign each section of the Application Kit.

Application for Coverage

Claim/Suit Information Addendum

Subscription Agreement for the Subordinated Surplus Deposit Certificate

Business Associate Agreement

2. MAIL or FAX your completed Application Kit to TMLT.

3. Submit your check for the Surplus Deposit along with your application. The Surplus Deposit must be paid and your application approved prior to binding or effecting coverage.

Definition of Surplus
Surplus is critical to the financial stability of an insurance company, as it indicates whether or not a company has adequate capitalization and financing to pay future claims. Surplus provides the cushion required to handle the inherent variability in estimating claims losses. Contributed Surplus represents contributions by policyholders, usually as a condition of coverage; Unassigned Surplus is that generated by operations.

4. Be sure to print out and review the Offering Circular contained in the Application Kit.
