Request Individual Quote

If you are a physician and you solely own your practice or you are employed independently, select the “individual quote” option. This option provides a pricing estimate for one physician only.

Select the “group quote” option if

  • your group practice is not currently insured byTMLT; or
  • you are requesting a quote for more than one provider who work at the same practice, operate as a group, and you’re seeking a quote for the entire practice

This option provides itemized pricing for each physician, each allied health professional (if applicable), and the entity (if eligible).
